Suggested Documents - If Applicable


  • Other Relevant Education
    Please submit photocopies of any diplomas, degrees, certificates if applicable e.g., MSc , MA , MEd, PhD.
  • License(s) to Practice Medicine Please provide Notarized/Certified photocopies of each license if applicable.
Photocopies of notarized documents are acceptable.

Document Authenticity

The following supporting documents require notarization by a Notary Public or certification by a Commissioner of Oath:

  • Proof of Name Change
  • Statement of Canadian legal status
  • Proof of Language Proficiency (unless ETS submits TSE-P and TOEFL results directly to CEHPEA)
  • Medical Council of Canada Examinations - Statement of Results
  • Undergraduate medical degree/diploma and transcripts
  • Postgraduate medical degree/diploma and transcripts (if applicable)
  • License to Practice Medicine in another jurisdiction (if applicable)

Please note that where notarized/certified photocopies are required, such verification is only authentic if it is stamped by a Notary Public or a Commissioner of Oath wherein the stamp clearly acknowledges such status.

Photocopies of the above notarized documents are acceptable.
