Mandatory Assessment Requirements for All Entry Levels




Below are descriptions of all required documents for the 2010/11 CEHPEA application form.


It is your responsibility to ensure that your application is complete before the deadline. Applications missing the application fee will not be eligible for assessment. Documentation to support applications that arrive after the deadline will not be added to the portfolio.

Curriculum Vitae

Please submit a current up to date Curriculum Vitae.

Personal Information

Photograph

In order to protect candidates' confidentiality and to ensure the integrity of their application, a personal photograph is required. All candidates must submit a photograph of passport size and quality. The photograph must be full face and taken within 60 days of the date of application.

Proof of Name Change

If a candidate is submitting supporting documentation for a name other than the one shown on the application form, proof of name change must be submitted in the form of a Notarized/Certified copy of one of the following: 1. Marriage Certificate or 2. Change of Name Order issued by an Ontario court judge, or 3. By Another Court of Competent Jurisdiction. Photocopies are not acceptable.

Canadian Legal Status

All applicants applying for assessment may apply to CEHPEA for assessment regardless of Ontario resident (or residency) status. A copy of one of the following may be submitted at the time of application to verify legal status in Canada:

  • Notarized/certified photocopy of Birth certificate with any photo ID
  • Notarized/certified photocopy of Canadian Passport
  • Notarized/certified photocopy of Canadian citizenship certificate, Record of Landing
  • Notarized/certified photocopy of Permanent Resident Card/Canadian Citizen Card
  • Application to Citizenship and Immigration Canada (CIC) for Permanent Resident status, or
  • A signed letter indicating intent to apply to immigrate to Canada. Please note that photocopies of notarized documents are acceptable.

Language Proficiency

All candidates must demonstrate fluency in English or French through ONE of the following (A, B, C, D, E or F):

  1. TSE-P Score Date, TOEFL Score Date, Essay Writing Score
    Please have ETS send TSE and TOEFL results directly to our program (Institution Code: 8316, Dept Code: 42), or please submit notarized/certified copies of TSE and TOEFL results.
  2. TOEFL-Ibt Score Date Writing Section Score
    Please have ETS send TOEFL-iBT results directly to our program (Institution Code: 8316, Dept Code: 42) or please submit notarized/certified copies of TSE and TOEFL results.
  3. Language Proficiency Form: Primary (PDF) and Secondary School (PDF)
    Mailed directly from the Senior Academic Administrators of both schools to the CEHPEA office confirming that all of your primary and secondary education was conducted completely in English or French.
  4. Language Proficiency Form: Medical School (PDF)
    Mailed directly from the Dean of the Medical School to the CEHPEA office confirming that your language of instruction and patient care was conducted completely in English or French.
  5. WHO / FAIMER Language Option If the language of instruction and the language of patient care at the undergraduate medical school was conducted completely in English or French then candidates can submit a copy or printout from the WHO website or book (World Health Organization Directory, 7th Edition) or the FAIMER listing the medical school which clearly states that the language of instruction is English or French.

    Candidates also have to answer the following question:
       - Was the language of patient care conducted completely in English or French? Yes/No
  6. A pass result in the French proficiency test administered by the Office québécois de la langue française (OQLF).
    Please submit notarized/certified copies of French proficiency test results.

Please note documents below that must be submitted:

  • Language Proficiency Forms will not be accepted from the candidate or a third party.
  • Language Proficiency Forms and/or results will not be accepted by fax or email.
  • TSE: Minimum score 50.
  • TOEFL: Minimum score 237, or minimum score 580 for paper-based test.
  • TOEFL-Ibt: Passing score is 93 including a minimum score of 24 on the speaking section.
Photocopies of notarized/certified documents are acceptable.

MCCEE

All applicants applying for assessment to CEHPEA for the 2010/11 application cycle must achieve a passing result on the Medical Council of Canada (MCC) Evaluating Examination. A notarized / certified photocopy of your MCCEE Statement of Results must be submitted to CEHPEA. Photocopies of notarized documents are acceptable.

MCCQE1

All applicants applying for assessment to CEHPEA for the 2010/11 application cycle must achieve a passing result of the Medical Council of Canada Qualifying Examination Part 1 (MCCQE1). A notarized/certified copy of the MCCQE1 Statement of Results must be submitted to CEHPEA. Photocopies of notarized documents are acceptable.

Medical Diploma

Academic Qualifications - All candidates must have:


  1. completed an undergraduate medical degree or
  2. completed an equivalent basic degree in medicine. The program must be a conventional undergraduate program of education in allopathic medicine that:

    • Teaches medical principles, knowledge and skills similar to those taught in undergraduate programs of medical education at an LCME/CACMS accredited medical school,
    • Includes at least 130 weeks of instruction over a minimum of 36 months, and
    • Was, at the time of the applicant's graduation, listed in the World Directory of Medical Schools published by the World Health Organization and/or FAIMER.

A Notarized/Certified copy of medical degrees/diplomas must be submitted.

Degrees/diplomas that are not in English must be accompanied by a notarized translation.

Transcripts

A Notarized/Certified copy of medical school marks must be submitted. Transcripts that are not in English must be accompanied by a notarized translation. Photocopies of notarized documents are acceptable.

**Ophthalmology Candidates Only – ICO Examination Results

Please submit your most recent ICO Examination results directly to CEHPEA.

Candidate Declaration

All applicants must complete, and electronically sign the application form. By signing the form the applicant attests that he/she:
  1. has read and agrees to abide by the terms and conditions of the CEHPEA assessment process;
  2. has disclosed or will disclose all information and documentation material to a decision with respect to the application, and the information and documentation submitted or to be submitted in connection with the application including the declaration as to citizenship and status in Canada (collectively, the "Application"), is true, complete and correct;
  3. has disclosed in the Application any previous loss of licensure or charge of professional misconduct;
  4. grants permission to CEHPEA to contact previous program directors, or any person or institution cited in the Application to obtain further information reasonably relevant to assessing the applicant's qualifications;
  5. consents to the collection, use, and disclosure by CEHPEA of all personal information contained in these application materials, or reasonably arising or resulting from the information provided in these application materials and the results of taking any and all examinations, for the limited purposes of:

    • processing the Application, compiling general statistical information, and administering any and all programs and processes under the auspices of CEHPEA;
    • providing information to medical schools for the purposes of such medical schools assessing the applicant's Application and qualifications for admittance to such medical schools;
    • providing information to program directors for the purposes of the interview and possible selection process;
    • providing information to the Canadian Post-MD Education Registry (a division of the Association of Faculties of Medicine of Canada) for purposes of producing databases of information (aggregated and identifiable) relating to International Medical Graduates in Canadian Post-M.D. training programs and in the Canadian physician workforce; and
    • providing information to CARMS (the Canadian Residency Matching Program) for purposes of determining eligibility for the residency matching program and, if appropriate, entry into and the administration of the residency matching program
  6. understands that:
    • if any information or documentation submitted in support of the Application is false or misleading, or if any material information has been concealed or withheld:
      • the Application will be invalidated which will result in its immediate rejection; and
      • the application fee will not be refunded;
    • it is applicant's responsibility to ensure that all documents are submitted in accordance with the assessment schedule, and all documentation submitted in support of the assessment becomes the property of CEHPEA and will not be returned;
    • if the application is incomplete it will not be eligible for assessment; and
    • CEHPEA assessment requirements and programs are subject to change without notice and that revisions and updates will be available on the CEHPEA website.
