Required documents that you will need to submit to CEHPEA
In addition to the documentation that you will submit to CEHPEA through the PCRC, we require you to supply the following documents when completing an application for CEHPEA examinations.
It is your responsibility to ensure that your application is complete before the deadline. All fees must be paid when you submit your online application or you will not be eligible for assessment. Please allow three business days for your application to be processed.
Curriculum Vitae

Photograph
In order to protect candidates' confidentiality and to ensure the integrity of their application, a personal photograph is required. All candidates must upload a photograph electronically under the Document Status menu after submitting your application with the following specifications:
- Must be current, i.e. taken within the past six (6) months.
- Must be color (black and white photos are not accepted).
- Must be passport-size, i.e. 50 mm (2 inches) wide by 70 mm (2 3/4 inches) long.
- Scanned photographs must be of passport quality, i.e. full-face and very clear with a contrasting background.
- Must be an original, i.e. not taken from an existing photo.
Proof of Canadian Legal Status
All applicants applying for assessment may apply to CEHPEA for assessment regardless of Ontario resident (or residency) status. A copy of one of the following must be uploaded electronically under the Document Status menu after submitting your application.
- Birth certificate with any photo ID
- Canadian Passport
- Canadian citizenship certificate, Record of Landing
- Permanent Resident Card/Canadian Citizen Card - Please scan both the front and back of your card and upload as one document.
- Application to Citizenship and Immigration Canada (CIC) for Permanent Resident status, or
- A signed letter indicating intent to apply to immigrate to Canada.
Candidate Declaration Form
All applicants must complete, and electronically sign the application form. By signing the form the applicant attests that he/she:
- has read and agrees to abide by the terms and conditions of the CEHPEA assessment process;
- has disclosed or will disclose all information and documentation material to a decision with respect to the application, and the information and documentation submitted or to be submitted in connection with the application including the declaration as to citizenship and status in Canada (collectively, the "Application"), is true, complete and correct;
- has disclosed in the Application any previous loss of licensure or charge of professional misconduct;
- grants permission to CEHPEA to contact previous program directors, or any person or institution cited in the Application to obtain further information reasonably relevant to assessing the applicant's qualifications;
- consents to the collection, use, and disclosure by CEHPEA of all personal information contained in these application materials, or reasonably arising or resulting from the information provided in these application materials and the results of taking any and all examinations, for the limited purposes of:
- processing the Application, compiling general statistical information, and administering any and all programs and processes under the auspices of CEHPEA;
- providing information to medical schools for the purposes of such medical schools assessing the applicant's Application and qualifications for admittance to such medical schools;
- providing information to program directors for the purposes of the interview and possible selection process;
- providing information to the Canadian Post-MD Education Registry (a division of the Association of Faculties of Medicine of Canada) for purposes of producing databases of information (aggregated and identifiable) relating to International Medical Graduates in Canadian Post-M.D. training programs and in the Canadian physician workforce; and
- providing information to CARMS (the Canadian Resident Matching Service) for purposes of determining eligibility for the residency matching program and, if appropriate, entry into and the administration of the residency matching program
- understands that:
- if any information or documentation submitted in support of the Application is false or misleading, or if any material information has been concealed or withheld:
- the Application will be invalidated which will result in its immediate rejection; and
- the application fee will not be refunded;
- it is applicant's responsibility to ensure that all documents are submitted in accordance with the assessment schedule, and all documentation submitted in support of the assessment becomes the property of CEHPEA and will not be returned;
- if the application is incomplete it will not be eligible for assessment; and
- CEHPEA assessment requirements and programs are subject to change without notice and that revisions and updates will be available on the CEHPEA website.
